It is most likely a pic of a normal one headed snake with the people in the background watching it, some one has just photoshopped it.

That photo of the cobra is photoshopped, there are snakes out there with more then one head it's a condition called polycephaly.

It's real, was only recently described as Naja tricephalus a species of cobra restricted to some 3rd world countries. I only just read the scientific paper and was finding it hard to imagine such a snake so it's great to see a photo of one. Think it was described by a Tasmanian taxonomist
